combination of experience and knowledge of a nurse is the greatest influence on
clinical judgment. This is demonstrated by Tanner when she stated that clinical
judgments need different types of knowledge, which is generalizable, applicable
and abstract in many conditions, and is generated from theory and science (Tanner,
2006). Tanner expounded on greatest influences on clinical judgement by reviewing
about two hundred studies and, came up with five conclusions, and provided an
alternative clinical judgment model (Tanner, 2006). From the studies, Tanner
found out that what nurses bring to the particular situation rather than objective
data influences clinical judgments. Moreover, knowing the sick person and his
or her response pattern by a nurse, and patients’ engagement with the nurse
form the basis of sound clinical judgment. The situational context and nursing
care unit culture affects clinical judgment. Nurses apply distinctive reasoning
patterns in combination or alone to solve a problem. Clinical judgment
breakdown always triggers reflection on practice. Furthermore, it is responsible
for clinical reasoning improvement and clinical knowledge development. Therefore,
Tanner suggested the clinical judgment model, which is a type of engaged ethical
reasoning. According to Tanner, an experienced nurse handles patients with an indispensable
sense of what is right and good (Tanner, 2006). She or he when attending to
patients has a vision for offering exquisite care. Tanner argued that
educational practices should help nurse students to engage with patients
(Tanner, 2006).
Role
of intuition in clinical judgment
Intuition
helps nurses in making decisions. It enables nurses to reason ethically. A
nurse develops nursing intuition when dealing with the particular situation of
the patient. For instance, the student nurse should be responsible for
providing superb care for those patients and at the same time considering the
well-being of patients and families of the patients. Therefore, this engaged
and concerned stance between the patients and nurses, and informed knowledge of
nurses should form the basis of clinical reasoning. However, it should not be
an objective for and separate exercise from the concerns of patients. Tanner
added that nurse educators should help nurse students comprehend, develop and
become moral agents (Tanner, 2006). This approach allows nurse students to expand
their clinical knowledge through coaching and guidance of their experts, and
become excellent in their nursing practices. As a result, they would think like
a nurse.
